- Includes everything in the DUTCH Complete — sex hormones, adrenal hormones, oestrogen metabolism pathways and organic acid markers.
- Adds the Cortisol Awakening Response (CAR) — an additional saliva assessment collected in the first 60 minutes after waking.
- In healthy individuals, cortisol rises by up to 50% within 30 minutes of waking and returns to baseline within 60 minutes. This is the CAR. A blunted or exaggerated CAR is associated with chronic stress, burnout, fatigue, depression and HPA axis dysregulation.
- Provides a more detailed picture of adrenal function than the DUTCH Complete alone.
- Sample type: dried urine and saliva — collected at home over one day.
- I recommend the DUTCH Plus when adrenal function, stress resilience or burnout is a central part of the clinical picture, rather than a secondary concern.
The DUTCH Complete includes free cortisol across the day and covers most women well. The Plus is specifically indicated when the CAR pattern is needed for a more detailed HPA axis assessment.
Tracks hormone patterns across the whole month rather than a single snapshot — the right choice if cycles are very irregular or ovulation tracking is the central question.
The Cortisol Awakening Response (CAR) and optional extra cortisol sample are included in the DUTCH Plus. CAR is collected via saliva — waking, +30 min and +60 min after waking.
Like the DUTCH Complete, urine samples are collected in the luteal phase — 5–7 days after ovulation. The DUTCH Plus adds saliva samples collected in the first 60 minutes after waking to measure the Cortisol Awakening Response — on the same collection day.
If you have regular cycles, timing is straightforward. If you’re post-menopausal, any day works.

On collection day, you’ll provide three saliva samples in the morning: immediately on waking, 30 minutes after waking, and 60 minutes after waking. Do not eat, drink or brush your teeth before the +60 minute sample. Freeze all saliva samples until you’re ready to ship. If you wake overnight, one optional saliva sample can also be collected at that time.
The DUTCH Plus is a single snapshot. If your cycles are irregular, you’re in perimenopause, or your symptoms change significantly throughout the month, DUTCH Cycle Mapping may be the better fit. Get in touch and I’ll advise.
If using an OPK to time collection, be aware that LH can fluctuate in perimenopause and may cause false positives. BBT tracking is more reliable for confirming ovulation at this stage.

I recommend the DUTCH Plus when adrenal function is a central concern alongside hormonal symptoms — particularly where burnout, chronic fatigue, poor sleep resilience or morning mood dips suggest the CAR pattern needs closer assessment. If adrenal function isn’t your primary concern, DUTCH Complete is usually the better starting point.
If your cycles are irregular, you’re perimenopausal, or your symptoms change significantly throughout the month, DUTCH Cycle Mapping may be the better option. Get in touch and I’ll advise.

You can also test while taking oral progesterone. Progesterone metabolites will usually show high, but I still advise taking it as you normally do — especially if you take it before bed for sleep or stress support.
If you’re on the oral contraceptive pill, wait 3 months after stopping before testing, so your natural hormone production has time to return.
The DUTCH measures hormone metabolites in dried urine collected across the day, giving a more complete picture of hormone production, metabolism, and clearance. It also measures cortisol at four time points, allowing you to see your cortisol rhythm rather than a single value.
In addition, the DUTCH includes oestrogen metabolism markers and organic acid markers that are not available on standard blood testing.
