Like the DUTCH Complete, urine samples are collected in the luteal phase — 5–7 days after ovulation. The DUTCH Plus adds saliva samples collected in the first 60 minutes after waking to measure the Cortisol Awakening Response — on the same collection day.

If you have regular cycles, timing is straightforward. If you’re post-menopausal, any day works.

Saliva collection — Cortisol Awakening Response (CAR)

On collection day, you’ll provide three saliva samples in the morning: immediately on waking, 30 minutes after waking, and 60 minutes after waking. Do not eat, drink or brush your teeth before the +60 minute sample. Freeze all saliva samples until you’re ready to ship. If you wake overnight, one optional saliva sample can also be collected at that time.

Perimenopausal, irregular cycles, or symptoms vary throughout the month?

The DUTCH Plus is a single snapshot. If your cycles are irregular, you’re in perimenopause, or your symptoms change significantly throughout the month, DUTCH Cycle Mapping may be the better fit. Get in touch and I’ll advise.

If using an OPK to time collection, be aware that LH can fluctuate in perimenopause and may cause false positives. BBT tracking is more reliable for confirming ovulation at this stage.

I recommend the DUTCH Plus when adrenal function is a central concern alongside hormonal symptoms — particularly where burnout, chronic fatigue, poor sleep resilience or morning mood dips suggest the CAR pattern needs closer assessment. If adrenal function isn’t your primary concern, DUTCH Complete is usually the better starting point.

Yes, you can do DUTCH testing while using HRT, including transdermal oestrogen gels or patches.

You can also test while taking oral progesterone. Progesterone metabolites will usually show high, but I still advise taking it as you normally do — especially if you take it before bed for sleep or stress support.

If you’re on the oral contraceptive pill, wait 3 months after stopping before testing, so your natural hormone production has time to return.

A blood test gives you a snapshot of hormone levels at a single moment in time. Hormones such as oestrogen and progesterone naturally fluctuate throughout the day, and cortisol follows a daily rhythm.

The DUTCH measures hormone metabolites in dried urine collected across the day, giving a more complete picture of hormone production, metabolism, and clearance. It also measures cortisol at four time points, allowing you to see your cortisol rhythm rather than a single value.

In addition, the DUTCH includes oestrogen metabolism markers and organic acid markers that are not available on standard blood testing.
